Step 1

Create an account

Sign up with your name, email, specialty and a password. The first month is free and no credit card is required.

You're signed in as soon as you register. Your portal is always at solo.medhistory.app — bookmark it.

Work at a clinic? You don't sign up yourself — your clinic's administrator creates your account, and you log in at your clinic's own address (see For clinics). Clinic not on MedHistory yet? Book a demo.

Step 2

Add a patient

On the home page, click “+ Add patient” and enter the first name, last name and date of birth. Everything else — diagnoses, medications, history — is filled in from the records.

Find patients by searching for their name. The pencil next to a patient edits their details; the bin deletes them along with all their records.

At a clinic, the front desk usually adds patients. When they check a patient in for you, the patient appears at the top of your home page, under “Waiting for exam”.

Step 3

Upload records

Open a patient, click “+” next to the tabs and choose “Import document”.

Drag and drop files

PDFs, scans and photos — several at once, up to 25 MB per file. Order and sorting don't matter.

Or take photos with your phone

“📷 Take photos with phone” shows a QR code. Scan it and photograph the paperwork — the pages go straight into the patient's record.

The system reads them for you

Click “Next: Read documents →”. Each document is recognized, dated and classified — usually within a couple of minutes.

Step 4

Review the timeline and summary

On the left of the patient's record are the Summary (diagnoses, current medications, trends in key values) and the History — every document in date order. Click a document to open it in the middle.

Transcript

Findings from the document, organized: diagnosis, procedure, medication…

Lab results

All results in a table, with out-of-range values flagged.

Original

The original scan or PDF — always available to check against.

Step 5

Review CT, MR and other scans

DICOM imaging (CT, MR, X-ray from the disc the patient brought) is uploaded like any other record: “+” → “Import document”, then drag in the whole folder from the disc or click “choose a folder”.

Upload the whole folder

Each study becomes an “Imaging” document in the History, with the date and description from the scan itself. Other files on the disc (DICOMDIR, viewer software) are skipped automatically. ZIP archives aren't supported — unzip them first.

Scroll and adjust

Click the scan in the History. The mouse wheel scrolls through slices, the left button adjusts contrast, the middle button pans and the right button zooms. At the top are “Series”, “Invert”, “Reset” and “Fullscreen”.

Compare two studies

“Compare” opens another study of the same patient next to this one — a follow-up CT next to the previous one, for example. Each side scrolls and adjusts on its own.

The AI doesn't read DICOM images — the Assistant only knows a scan exists, not what it shows. If the name in the scan doesn't match the patient's name, the viewer warns you.

Step 6

Use the AI tools — and check them

On the right of the record is the Assistant. Ask it about the patient — it answers from the uploaded records, and the numbers [1], [2] in an answer are its sources: click one to open the exact document with the relevant data highlighted.

Before using it for the first time, you confirm a one-time consent: the AI is only an aid that makes suggestions, it can make mistakes, you check every fact against the original, and the final decision is yours. Every AI answer carries the label “⚠ AI-generated”.

  • How did the medication change after the heart attack?
  • Which results are outside the reference range?
  • When was this diagnosis first documented?

Next to the Assistant are DDx (possible diagnoses, with reasons for and against) and Validation (conflicting and missing data, medication checked against the drug registry). The AI tools are for doctors only.

Step 7

Record the conversation with the patient

Click “+” → “Conversation recording” and scan the QR code with your phone. On the phone, tap “Record” and leave it on the desk with the screen on — the transcript appears on your computer as you talk.

After “Finish recording”, click “Create anamnesis” in the Anamnesis tab: complaints, present illness, medication, allergies… “Send to report” opens a new report with the anamnesis for you to review and complete.

Before you start, tell the patient that the conversation is being recorded. The recording and transcript are kept in the patient's record. The anamnesis is an AI-generated draft — read it before you use it.

Step 8

Write a report and add it to the record

Every patient opens on the Report tab. Write freely — everything saves automatically. “Insert from record” adds diagnoses or medications in one click.

You write the report from a template — a built-in one or your own, made on the “Templates” tab of your home page. “Add to record” files it in the patient's history — and if the patient was checked in for an exam, that also finishes the exam. “Copy” lets you paste it into the software you already use.

For clinics

Setting up your clinic

A clinic has its own address and three kinds of accounts: the clinic administrator, doctors and the front desk. This part is for the administrator — what to set up before doctors and the front desk start working.

Clinic · step 1

Add doctors and front-desk staff

Everyone logs in at your clinic's address, yourclinic.medhistory.app, using the same form. Add a doctor under Doctors → “+ Add doctor” (name, specialty, email, phone and password), and front-desk staff under Front desk → “Add receptionist”.

Clinic administrator

Adds doctors and front-desk staff, sets working hours, and sees all patients, appointments, the activity history and exports. Doesn't use the AI tools or run exams.

Doctor

Sees their own patients and the ones they've examined, writes reports and uses the AI tools. Their home page has “My appointments” and “Time off”.

Front desk

Adds patients, books appointments, checks patients in and uploads records. Doesn't write in the record or use the AI tools.

No email is sent to new accounts — you set the password and pass it on to the doctor or receptionist, together with the clinic's address. Deactivate an account you no longer need (“Deactivate”); its patients stay with the clinic.

Clinic · step 2

Set working hours

Under Doctors, click “Schedule” next to a doctor. Only the clinic administrator can change working hours.

Usual week

Drag in a day's column to draw a shift, then move it or change its length. Quick options are “08–16”, “Morning 07–14” and “Afternoon 14–21”. Save with “Save working hours”.

Monthly schedule

For exceptions, select days in the calendar, enter From/To and click “Set hours” — or “Off” for a day off. “Back to usual week” undoes the exception.

Appointment length

On the same page, choose the doctor's default appointment length (10–60 min). It applies to every new appointment and to walk-in patients.

If new hours leave existing appointments outside a shift, they aren't moved automatically — you get a warning listing them, so you can move them in the calendar.

Clinic · step 3

Add time off

The administrator adds time off on the doctor's “Schedule” page, in the “Time off” card. Doctors can add their own time off in the “Time off” tab on their home page.

Click “Add time off”, choose the dates, the whole day or part of it, and a reason (vacation, sick leave, conference…). During time off the doctor can't be booked without an explicit override.

Appointments already booked in that period aren't cancelled automatically — you get a warning with how many there are, so you can move or cancel them.

Clinic · step 4

Book appointments

Appointments is the calendar for all doctors, by day or week. Click a free slot in a doctor's column — or the “Appointment” button — to open “Book appointment”: find the patient (or add them on the spot), pick one of the free times and click “Book”.

Drag an appointment to move it; click it for “Check in”, “Edit”, “No-show” or “Cancel appointment”. The calendar updates live for everyone who has it open.

A doctor can never have two appointments at the same time. Booking outside working hours or during time off is only possible with “Allow booking outside working hours” ticked.

Clinic · step 5

Check patients in for their exam

When a patient arrives, the front desk clicks “Check in” — in the calendar or in the Patients list — and chooses “Check in on appointment”, or, without an appointment, “Check in as walk-in” with one of the doctors.

Front desk

The patient joins the doctor's queue as “Waiting for exam”. If they were checked in by mistake, “Remove from exam” undoes it.

Doctor

The patient appears under “Waiting for exam” at the top of the home page. The exam starts with the first thing typed in the report, or with “Start exam”.

Finishing the exam

“Add to record” files the report and finishes the exam, and the appointment is marked Completed. An exam with nothing to write is closed with “Finish exam”.

Clinic · step 6

Profile, activity history and export

Clinic profile

Name, address, tax and registration numbers, logo and footer — everything printed in the letterhead of every report.

Activity history

Who changed what, who opened which document and when, and who accepted the AI tools consent — filterable by patient, user and date.

Data export

Appointments and exams, doctors, patients, documents and files, as CSV — for all patients or one.

FAQ

Have a question?

Which formats can I upload?

PDFs, scanned documents and photos (JPG, PNG, HEIC), up to 25 MB per file, and DICOM imaging (CT, MR, X-ray) from a disc or folder. Handwriting is recognized if it's legible; for best results, photograph the page flat and in good light.

What if the AI makes a mistake?

It can happen — that's why every piece of data in the summary, the timeline and the assistant's answers is linked to the original document. Click the source and check. The AI is an aid that makes suggestions, and medical decisions are always yours.

Does the AI interpret CT and MR scans?

No. DICOM images are never sent to the AI — you review them yourself in the viewer. The Assistant only sees that a scan exists, and reads the radiologist's written report like any other document.

Who can see my patients?

If you work on your own — only you. At a clinic, the clinic administrator and the front desk see the clinic's patients, and a doctor sees the patients they're the chosen doctor for, have an appointment with or have examined. Notes are always private. Data is encrypted and separated by clinic, and MedHistory uses it for nothing other than your work.

How does a clinic doctor get an account?

The clinic administrator creates it under Doctors. No email is sent — the administrator gives you the clinic's address and your password, and you log in at your clinic's address.

Do I need the patient's consent?

Yes — you upload only records for which you have the patient's consent or another legal basis. Before recording a conversation, tell the patient it's being recorded. Details are in the Terms of Service.

Do I have to replace the software I already use?

No. MedHistory works alongside your existing software — transfer reports with the “Copy” button.

What happens after the trial?

The trial lasts 30 days, with no credit card. Before it ends, we'll let you know the terms for continuing — if you don't continue, you aren't charged anything, and you can export your data.

How do I delete a patient or my account?

Delete a patient with the bin icon on the home page. To close your account or export your data, email us at [email protected] — clinics can also export their data themselves, under Data export.

Didn't find your answer? Email [email protected].
